Public Discrimination Reporting Form

NPS is committed to maintaining an education and work environment for all school community members that is free from all forms of discrimination, harassment and retaliation. 

If you are a student, staff member, or parent and would like to report an incident of alleged discrimination by an NPS community member, you may either contact the school principal (assistant principal or dean) or complete this form. 

The information you provide will be forwarded to the NPS Department of Diversity, Equity and Inclusion for follow-up and potential investigation.
